Your Insurance Coverage May Be Terminated
We have received calls from members who believed recent letters, phone calls, or emails about dependent verification were a scam. Please know these communications are legitimate and are required by the PAMCAH– UA Local 675 Health & Welfare Plan. Letters, phone calls, and emails are conducted by Part D Advisors (PDA). If you receive these notices, you must respond to avoid your dependent coverage being TERMINATED.
Complete Your Dependent Verification
As a reminder, the PAMCAH-UA Local 675 Health & Welfare Plan (the "Plan") has been conducting a dependent verification process to confirm eligibility for dependents covered under the Plan. Part D Advisors (PDA) was contracted to manage this process, which began on July 21, 2025.
Multiple reminders — including letters, phone calls, and email follow-ups — have been sent to members who have not yet responded or who still have missing documentation.
Participation in the dependent verification process is mandatory. If the required documents are not submitted, coverage for your dependent(s) will be terminated.* To avoid termination of coverage and potentially significant out-of-pocket expenses, please ensure you complete the dependent verification requirements as soon as possible.

* Please note that coverage can only be reinstated on a prospective basis once all required documents have been provided. Any healthcare claims incurred during a termination period will not be covered by the Plan and will become your financial responsibility.
